- Membership of the Fit 4 Future programme is for a limited period up to a maximum of 12 months between the period 1st April 2009 and 31st March 2010.
- Participants are subject to and must abide by the terms and conditions of membership which are applicable to the Gym/Leisure Centre joined.
- Membership subsidies are subject to regular attendance (average of one visit per week over a 3 month period) by the participant. Failure to achieve the required attendance levels will result in the participant’s subsidy being lost and membership reverting to the Fit 4 Future agreed rate of either £15 or £20per month.
- Monthly membership fees are payable either by cash or Direct Debit mandate. Cash payments can be made either quarterly (£15) or on a monthly (£5) basis. Cash payments must be received within 5 days of the previous payment expiry date. Failure to do so may result in your membership being suspended and possibly withdrawn.
- Current members of a Gym/Leisure Centre are not eligible to apply to rejoin the facility under the Fit 4 Future programme within 6 months of membership cancellation.
- Access to all activities and facilities are subject to the Gym/Leisure Centres normal programme timetable.
- You may cancel your membership at any time during the Fit 4 Future programme without incurring any financial penalties. If you do cancel your membership you will not be eligible to rejoin the Fit 4 Future programme.
- As part of the Fit for Future Pilot programme you are required to complete an induction questionnaire and an exit survey. This information will be collated and used to both measure and evaluate the success or otherwise of the programme.
- Please note that in signing up to the Fit4Future scheme, you agree that the information you've supplied will be used in confidence to evaluate the scheme and will be passed in confidence to members of the research team. As part of the Fit4fFuture scheme's evaluation, you may be contacted directly by phone or email about your experience up to 30 June 2010
